Primary Work Location: 

Float throughout two locations in East Bay. 

 

Job Summary:   

The care coordinator will assist clinical staff and residents in the coordination of medical and behavioral health care. 



Qualifications

Job Duties 

  • Maintain a calendar of upcoming resident medical and behavioral health appointments. Communicate upcoming appointments to clients, clinical staff, and transportation services 

  • Assist clinical staff and site-based residents in the scheduling of client medical and behavioral health appointments. 
    Communicate with local pharmacies regarding site-based resident prescriptions. At the direction of nursing staff request medication refills, deliveries, and transfers 

  • Request relevant medical records of site-based residents from hospitals and community clinics. Transmit medical records and other authorized requested documentation to partner clinics to facilitate care coordination 

  • Interface with site-based resident healthcare coverage plans to avoid interruption in services and facilitate care 

  • Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls 

  • Ensure adherence to infection control standards on site among staff and residents 

  • Answer calls to the nursing station and respond independently to when appropriate 

  • Assist nursing staff with light clerical duties including filing and making copies. 

  • Maintain confidentiality in accordance with the Health Insurance Portability and Accountability Act (HIPAA) 

  • Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect 

  • Other duties as assigned 

  • Constant work around and/or with people OR Interaction with patients, clients, and external customers 

  • Clinical setting in a permanent supportive housing site  

 

Minimum Required Skills & Qualifications:  

  • High School Diploma or equivalent 

  • Must be authorized to live and work in the U.S 

  • Must be at least 18 years of age 

  • Must have personal, reliable transportation to and from work sites and shifts

 

Preferred Skills & Qualifications:  

  • Prior experience with homeless or incarcerated populations is desirable 

  • Self-directed and able to work with minimal supervision 

  • Detailed oriented and highly organized 

  • Must provide proof of COVID vaccination or willing to be vaccinated 

  • Prior experience with homeless or incarcerated populations is highly desirable 

  • Proficiency in Microsoft Outlook, Word and Excel 

 

Physical Requirements:  

  • Occasional sitting or stationary work 

  • Frequent standing, walking, kneeling, stooping, and ascending stairs 

  • Occasional lifting and carrying up to 20 lbs 

  • Frequent pushing and pulling 

  • Frequent reaching high, low, and level 

 

Work Environment:  

In-person onsite at a supportive housing facility. Constant work around and/or with people OR Interaction with patients, clients, and external customers.
